Bringing Care to the Doorstep of the Community

The Mobile Health and Wellness Program is an innovative, collaborative care coordination model that aims to improve health outcomes and well-being of communities, provide access to services and educate future practitioners. The program brings services to participants at or near their place of residence and is centered on assessment, chronic disease monitoring, wellness coaching and care coordination.

Empowering Holistic Health and Wellbeing

In addition to basic health monitoring and assessment, our program addresses issues related to falls and mobility, screening for behavioral health issues including depression, anxiety and cognition, high-risk medication usage, and risk reduction.

The program also coordinates services affecting health and well-being through collaboration with our partners. Services include food access, health services, legal resources, wellness activities, enrichment programs and more.

Teaching Culturally Competent and Inclusive Care

At each site, VCU students, precepted by faculty from the VCU schools of Nursing, Pharmacy, and Social Work, as well as the departments of Occupational Therapy, Physical Therapy, Kinesiology and Health Sciences, and Psychology, meet with individuals to understand their health or social concerns. Together, the participant, student, and faculty develop a collaborative plan of care customized to the health conditions, social needs, and resources of each individual.

Collaborating on Research

Our program actively engages MHWP participants in community-engaged research, ensuring new studies address issues important to the communities we serve. This collaborative approach integrates participant insights and perspectives, guiding the direction, implementation, and impact of new research. As a result, findings are more relevant and actionable, bringing new solutions to address the specific needs and concerns of the community.
